A dear friend in her 80's still gets diarrhea problems,
5-6 years after her uterine cancer treatment.

Is this to be expected?
What's the mechanism/explanation?

I'm not an expert, but my father was treated for prostate cancer many
(10?) years ago, and still has diarrhea from it, as well as other
problems. They just did the radioactive "seeds" for him, which is
supposed to be the least invasive, best tolerated method. Apparently,
the radiation caused long-term damage in more than just the cancer
cells. HTH.
